Extracorporeal Shockwave Therapy (ESWT) is a new form of treatment for pain related to a number of chronic soft tissue conditions (eg, Achilles tendinopathy, plantar fasciopathy, calcific bursitis of hip & shoulder, tennis/golfers elbow, myofascial trigger points). This form of treatment was developed from other medical indications (eg. treating kidney stones) and is widely used in Europe and North America for the treatment of a number of soft tissue conditions. There are numerous published scientific studies, with the majority demonstrating positive outcomes of around 60 to 80%. More recently, research has shown that when used in combination with good rehabilitation, ESWT has better outcomes than when either treatment is employed alone. Most importantly ESWT has been demonstrated to be a safe treatment modality. Radial shockwaves are high energy soundwaves transmitted from a probe held against and passing through the skin and spreading outwards into the underlying tissues. The shockwave is a physical sound wave "shock,” not an electric one. This is believed to induce increased blood flow and metabolic activity around the site of pain accelerating the healing process & releasing your good pain relieving drugs into the body. ESWT promotes the remodelling of dysfunctional collagenous tissues. Shockwaves break down scar tissue and/or calcification. Transmission of pain is diminished through neurological mechanisms (inhibition of pain receptors).
ACCELERATED TISSUE REPAIR AND CELL GROWTH
Ingrowth of Neovascularization
Nutrient blood flow is necessary to start and maintain the repair processes of damaged tissue structure. The application of acoustic waves creates capillary micro-ruptures in tendon and bone and significantly increasing the expression of growth indicators such as the eNOS, VEGF, PCNA and BMP. These two processes stimulate the growth and remodeling of new arterioles. The new blood vessels will improve the blood supply and the oxygenation resulting in the faster healing of both tendon and bone.
Reversal of Chronic Inflammation
Chronic inflammation occurs when the inflammation response is not completely halted. It can damage healthy areas in the body and can result in chronic pain. The activity of mast cells, which are involved in inflammatory process, may be increased by pervasive acoustic waves. Mast cell activation may be followed by the synthesis of chemokines and cytokines. The releasing of pro-inflammatory compounds, when needed, may help to restore the normal healing and regenerative processes.
Stimulation of Collagen
The production of sufficient amounts of collagen is a necessary precondition of the tissue repair process. Shockwave therapy hastens procollagen synthesis. The newly created collagen fibers are forced into a longitudinal structure. These newly formed tendon fibers are more dense and stiff
